to use the spreadsheets:
Your agency will receive customized forms via e-mail
from your King County contract monitor. These customized
forms will contain your agency's contact information
and services/outcomes information specific to each King
County contract. E-mail completed forms to your contract
monitor as detailed in the Reporting Requirements section
of your contracts.
