My Home, My Life
The My Home, My Life is a pilot project funded by County DDD and sponsored by the Washington Initiative for Supported Employment. This project uses the principals of person centered planning to assist young adults with developmental disabilities to plan for community living options. These individuals might have limited or no funding for services. Individuals who want to plan and implement a move into the community receive assistance with building support teams, planning the housing process, maximizing service dollars, individualizing solutions to in-home supports, and accessing alternative supports.
