Homeless Outreach, Stabilization and Transition (HOST) Project
The Homeless Outreach, Stabilization and Transition (HOST) Project is a program operated by the Downtown Emergency Service Center (DESC) that provides outreach, engagement, stabilization and transition services to homeless persons with serious mental illness or co-occurring serious mental illness and substance abuse/chemical dependency.
Program Goal: The goal of the program is to ensure that homeless persons with serious mental illness or co-occurring serious mental illness and substance abuse/chemical dependency receive mental health services.
Eligible Population: The program serves homeless persons, age 18 years or older, with a serious and persistent mental illness or co-occurring serious mental illness and substance abuse/chemical dependency that are unable or unwilling to access mental health services and are not receiving other mental health services.
Service Area: The HOST Project provides services in downtown Seattle, North Seattle and East King County.
Services Provided: The HOST Project provides outreach and engagement services; assessment; access to survival services such as food, clothing, shelter, showers, and laundry facilities; intensive case management; psychiatric services; substance abuse/chemical dependency screening and assessment; referrals for primary health services; employment services; family support and education; housing services; and transition to long-term care in the Mental Health Plan (MHP) or other appropriate ongoing services.
Drop-In Center: DESC provides an informal drop-in space for clients that includes lavatories, showers, laundry facilities, access to computers, and storage. The Drop-In Center is open Monday through Friday from 9:00 a.m. to 12:00 p.m./noon.
