Public Hospital Dist. No. 2 - Proposition No. 1

BALLOT TITLE

PROPOSITION NO.1
PROPOSED ANNEXATION OF DUVALL AND THE
SURROUNDING COMMUNITY TO PUBLIC HOSPITAL DISTRICT NO. 2

Shall the City of Duvall and the community surrounding Duvall, such area being legally described in Resolution No. 639-00 adopted on August 22, 2000 by the Board of Commissioners of King County Public Hospital District No. 2 ("Evergreen Healthcare"), be annexed to Evergreen Healthcare?

Approved __   Rejected __

EXPLANATORY STATEMENT

The proposed annexation of the City of Duvall and the community surrounding Duvall will be approved if a majority of the votes cast by qualified voters living within the area proposed to be annexed are cast in favor of the annexation. If approved, the area described in Resolution No. 639-00 would become a part of King County Public Hospital District No. 2 whose purpose is to provide hospital and other health care services to residents of the District and to other persons.

STATEMENT FOR

The Problem

Medical services are not conveniently available to Duvall. Past clinics have come and gone leaving residents high and dry. A YES vote to annex Duvall to Evergreen's hospital district will provide permanent, stable services-a medical facility in Duvall, providing urgent care, operated by Evergreen Hospital Medical Center.

Who needs it? - Working people with children trying to get to and from doctors, compounded with traffic delays. - Isolated elderly for whom long distance means inadequate care. - Folks without a physician, perhaps new arrivals; - Anyone who might have a medical emergency when weather is bad.

What Would it Cost? Around $75.00 per year. An extremely good value-about the cost of two videos or one medium pizza per month. Increases are limited by state law!

What Would Annexation Bring? A local medical facility integrated with Evergreen's comprehensive system-known for quality, people-centered service. Less time and money spent commuting to other medical facilities. Opportunity to know your doctor as a neighbor.

Who Wants It? 30 local residents spent over a year studying the best medical care solution for Duvall and determined that a medical facility provided by Evergreen was it. In a survey of local Duvall residents, 80% agreed.
