Individual Action Plan

After taking the wellness assessment, employees and covered spouse/domestic partners receive the benefits of an individual action plan tailored to his/her level of assessed risk for developing a chronic health condition. By participating in the individual action plan, individuals earn the gold out-of-pocket expense level.

Low risk

For low-risk individuals, the individual action plan involves keeping a log of physical activity through the "Get Fit on Route 66" log or a log of nutritional choices through the "Colorful Choices" log for 56 consecutive days.

Moderate to high risk

For medium-risk or high-risk individuals, the individual action plan involves taking three coaching calls and engaging in at least one healthy activity before June 30.

Someone who is medium risk has the option to receive six months of coaching. Someone who is high risk may receive 12 months of coaching. Choosing to benefit from the additional coaching is entirely up to each individual.
