Important Deadlines
March 31, 2008
If you took the wellness assessment by Feb. 29, 2008, the next step toward earning gold is to complete your individual action plan. If you do not hear from Healthways (formerly Harris HealthTrends) by March 31, 2008, call them toll-free at 1-877-279-0624 and ask about your individual action plan options.You and your spouse/domestic partner have separate individual action plans based on your individual health risk. Since you and your spouse/domestic partner may have different health risk levels, you may have different individual action plans. Be sure to follow your own individual action plan.
May 15, 2008
If you have been identified as having a low health risk, you must begin your low-risk log (online or on paper) no later than May 15, 2008.June 30, 2008
You must complete your individual action plan by June 30, 2008 to earn the gold out-of-pocket expense level for your 2009 medical benefits.-
If you have been receiving coaching calls, your last call must be completed by June 30.

If you have been working on a low-risk log, your online log must have 42 consecutive entries by June 30. If you have been completing your log on paper, you must send your completed log to Healthways via fax at 1-888-572-5781 or mail (postmark no later than June 30) to Healthways 6629 West Central Avenue, Toledo OH 43617.
Low Risk
This year, you have three choices for a low-risk action plan designed to help you maintain or improve your healthy lifestyle:- Passport to Health. New this year, take a virtual trip around the world. As you tour exciting destinations, you’ll learn how to incorporate fitness and nutrition into your life. .
- Colorful Choices.No calorie counting, weighing or measuring. Simply track daily servings of fruits and vegetables.
- Get Fit on Route 66. This exercise program is designed to help you get fit and have fun. Just log the time and type of physical activity you do daily.
Moderate or High Risk
If your wellness assessment shows you are at a moderate or high risk for developing health problems, your individual action plan is to participate in the Healthways phone coaching program.Your action plan consists of three coaching calls where your coach reviews your goals and progress, offer tips and suggestions, and provides encouragement and feedback.
Within 30 days after completing your 3rd coaching call, you will receive a letter from Healthways confirming your 2009 medical out-of-pocket expense level (gold, silver, bronze).
