It is vital that we (Health Reform Initiative staff) hear how you’re doing with the Healthy Workplace Funding Initiative. We are here to help, to listen to your suggestions and to make changes for future years. Specifically, we would like to know how your instructors are working out, how many employees are attending classes or seminars, if you’re able to purchase the items you need and how this website serves your needs. Class Evaluation Form – Employee version – pass this out to employees at a class or seminar for their feedback. If you are holding multiple sessions of the same topic (i.e. a recurring yoga class) just one form per series is sufficient.
Contractor Evaluation Form – Coordinator version – coordinators use this to provide feedback on vendors.
