Important Information about the Wellness Assessment

By now, most of you should have received your wellness assessment letter in a salmon-colored envelope. If you and your spouse/domestic partner haven’t received your letters, contact Benefits and Retirement Operations (BROS) at 206-684-1556 or kc.benefits@metrokc.gov.

(NOTE: If you are a retiree, deputy sheriff or a member of the Technical Employees Association, you will not receive a wellness assessment letter because you are not participating in the Healthy Incentives program at this time.)

Your letter contains instructions and your personal access code for taking the assessment online. Each person’s access code is different, including your spouse/domestic partner’s access code. If you let someone else use your personal access code, you won’t be able to use your code to take the assessment online and you may not get credit for taking the assessment.

If you are not able to take the assessment online, you will need to contact BROS by January 18 to get a paper copy of the assessment sent to you.

Remember, you must take the wellness assessment online or send a completed paper version of the assessment to HealthMedia® by January 31, 2006, as your first step toward reaching the gold out-of-pocket expense level for 2007 medical benefits.
