Permit applications are now available on our website below. You will no longer need to obtain application forms at the permit counter. Instructions to assist customers in filling out the application forms are also available.

Please note that if you are applying for a Backflow Prevention Assemblies Permit for an installation within the City of Seattle you must also fill out the City of Seattle Backflow Prevention Assemblies Application Information Form and present it with your application submittal. These new application forms and instructions will be available at the permit counter also.

Instructions for use: When opening the Word version of the applications, it is formatted for you to tab to each field and type your information directly on the form. Once you are complete, print it out and fill out the rest that require a signature. The Adobe PDF versions are not formatted for direct typing. You will need to print out the PDF and fill in the blanks by hand.
