Screening forms

When a health care provider reports a suspect case of SARS to Public Health-Seattle & King County, the SARS investigator will need to collect clinical, as well as epidemiological information about the suspect case. Public Health is sharing our SARS screening forms so that health care providers who report suspect SARS can be prepared to provide some of this information when they call.

Should the World Health Organization determine that there is ongoing community transmission of SARS occurring in the world, Public Health will use the "Yes-SARS" screening and supplemental forms, otherwise, Public Health will use the "No-SARS" forms.

The case classification worksheet is used by Public Health to categorize the suspect SARS case using the information collected in the screening form. Please note that a suspect SARS case is classified using a combination of clinical and epidemiological criteria.
